Sometimes, a bed matters a lot!  Maybe a bed has never occurred to you as a potential birthday gift, but there is nothing more important than the place where you spend 7 to 10 hours of every day.
Shop around to get the best price and try to plan to have it delivered on the big day when your special someone is at work. Get a giant bow and wrap it around the mattress. If you have any extra money, throw in some new, fancy sheets.
